Employees departing January 1, 2026 and later, please complete the online form below. The completed form is automatically sent to the HR Performance for documentation of your responses.
You can print or save a copy for your records by clicking on the 3 dots (...) located in the upper right side of the screen of the completion message to access the print menu option.

Ethics Hotline - Whistleblower Policy
If a departing employee is not comfortable completing an Exit Questionnaire Form with their employee information, and they have reportable issues as outlined in the Whistleblower Policy (G1.25); they can access the Ethics Portal to submit an anonymous report online.
