Complete and submit this form in order to secure or decline your position in the Missouri State University Physician Assistant Studies class scheduled to begin in January 2027. This form must be submitted no later than the decision deadline listed in the acceptance letter.

By accepting a position in the program, you acknowledge that you have read, understand, agree, and comply with the program's admission requirements, curricular requirements, and technical standards that are detailed on the program's website. As a reminder, this includes the mandatory new student orientation that will be held on January 14-15, 2027. MSU PA courses will begin January 19, 2027.

You acknowledge that if you anticipate a barrier posed by any of the requirements above due to a disability, you will request an accommodation through the Disability Resource Center (DRC), Meyer Library, Suite 111, 417-836-4192. The DRC will engage in an interactive process to determine reasonable accommodations. You acknowledge that you may be asked to provide additional information, including relevant medical documentation.
