Social Security Number or Legal Name Change

A request to change your social security number or legal name in the Missouri State University System must be made at the appropriate office.

If you are an employee of the University, contact Payroll to report an incorrect social security number. In order to change your legal name on your personnel and payroll records, please refer to the Employee Change of Address/Name Form.

If you are a student, please refer to the Student Information Change form to change your SSN or legal name.
