Release of Information Authorization

A student may authorize the University to disclose education records that are otherwise protected under FERPA. Student records are maintained in various offices throughout campus, and therefore, you will need to submit a separate request to each relevant office. The offices that parent/guardians typically contact are Financial Aid, Financial Services, and Registrar.

Financial Aid

Students seeking to authorize the release of financial aid information to a designated individual or organization must complete the Financial Aid form. On the left-navigation bar, select "FERPA Release."

Financial Services - Account Detail

Students seeking to authorize the release of financial account information to a designated individual or organization must complete the Financial Services form. Go to the Accounts Receivable website and select "FERPA Release Form."

Registrar

Students seeking to authorize the release of educational records maintained by the Office of the Registrar to a designated individual or organization must complete the Registrar FERPA Release of Information Authorization Form. These educational records include, but are not limited to, registration, grades, tuition and fees, degree audit, and academic standing.

General Department Use

Students seeking to authorize the release of information from any other office should download the standard Release of Information Authorization Form and submit the completed, signed, and dated form to the appropriate office to whom you are authorizing to release specific education records. Forms are accepted in person, from the student’s MSU email account or by mail or fax.
