Student Forms

  • Academic Renewal Request form is used to invoke Academic Renewal. Important Note: If you choose Academic Renewal, you may be ineligible for financial aid. Check with the Financial Aid Office before deciding if Academic Renewal is right for you.
  • Address, Change of form is used to change mailing address, email address, phone number, and emergency contact information.
  • Audit, Request to place course on form is used to place a class on "Audit" basis.
  • Certificate, Reissue of form is used to request a reissued diploma or University Certificate. Current graduation candidates should not use this form unless they wish to order an additional copy of their diploma.
  • Degree Program, Application to form is required for a student to be admitted to their major or minor degree program.
  • Diploma/Certificate, Reissue of form is used to request a reissued diploma or University Certificate. Current graduation candidates should not use this form unless they wish to order an additional copy of their diploma.
  • Drop Exception Appeal Request form is used to request an exception to the drop policy by students who have an overwhelming, unforeseen circumstance which significantly interferes with their ability to meet their academic commitments (e.g., personal medical condition, catastrophic loss). A letter of explanation and supporting documentation as listed on the form are required.
  • Enrollment Verification Request form is used to obtain a letter or enrollment certificate to print and mail to insurance companies, lenders or other third parties.
  • FERPA Hold-Request To Prevent Disclosure of Student Information form is used when students wish to prevent disclosure of any information from their record to University officials and others (including the student's advisor) except as authorized by FERPA.
  • FERPA Hold Removal form is used to remove a FERPA Hold.
  • FERPA Letter of Recommendation form is used to allow the release of FERPA protected information to be included in a letter of recommendation by faculty member.
  • FERPA Release of Information Authorization form is used when students wish to request the release of FERPA protected information.
  • Mixed Credit form is used by students who have been admitted into an accelerated masters program to obtain approval to register for a graduate-level course which will count toward both their undergraduate and graduate degrees.
  • Name Change form is used when the student wants to change their official name at the University. Supporting documentation is required as stated on the form.
  • Overload Permission form is used to request permission to enroll in a number of credit hours that exceeds the University's limit for a semester/session.
  • Pass/Not Pass form is used to place a class on a Pass/Not Pass basis.
  • Personalized Minor Form form is used by the department to approve a personalized minor which must include the required courses and a specific title for the minor. Upon receipt of the form, the courses are entered on the student's degree audit.
  • Refund Exception Appeal form is used to request an exception to the refund policy.
  • Registration Request/Change of Schedule form is used to initially register, add, or drop classes.
  • Residence Requirement for Majors/Minors, Appeal to Waive form is used to request a waiver of the residence requirement for majors and minors.
  • Senior Permission for Graduate Credit form is used by MSU seniors and visiting seniors to obtain approval to register for a graduate-level class.
  • Student Information, Change form is used to change social security number, birth date, marital status, parent/guardian/spouse, and citizenship status. 
  • Substitution, Waiver, or Advised Course List form is used to substitute a course required for a student's degree program with an alternate course. It is also used if a course waiver is approved or if an advisor-approved list of courses is required.
  • Transcript Request Form is used to have an official transcript of your Missouri State coursework sent to yourself or to a third party.
  • Transfer Credit Reevaluation and Preapproval The new Transfer Credit Reevaluation and Preapproval online systems are designed so that current students and faculty/staff can appeal equivalency decisions or to seek equivalency decisions for transfer coursework. The online processes can be accessed through your my.missouristate.edu portal if you are a current admitted Missouri State University student or if you are a faculty/staff member who has equivalency authority.
  • Veterans Certification Request form is used by veterans who are requesting for their enrollment to be verified for benefit purposes.
  • Withdrawal Request form is used to drop all classes for the semester.

Faculty/Staff Forms

Paper Forms

  • Acknowledgment of Confidentiality Regulations is a standardized form a department may use as a written agreement for student employees who have access to private/confidential information.
  • Administrative Withdrawal due to Class Disruption form is used by the instructor to request an administrative withdrawal of a student due to class disruption.
  • Authorization to Repeat form is only used to authorize using a substitute course for a subsequent repeat or to grant permission to exceed the repeatable limit of a course.
  • Course Permission form is used to grant a student permission to register for a class. Departments and faculty are very strongly encouraged to grant permissions through Banner INB rather than using this paper form. For instructions go to: Experts website on Granting Overrides. This form gives permission to register but not does not complete the student's registration for the class.
  • Incomplete Grade, Assignment of form may be used by the instructor to indicate what a student must do to complete the class and how the completed work would affect the final grade.
  • Institutional Examination Report form is used by academic departments to award credit for passing a departmental examination.
  • Mixed Credit form is used to grant approval for students in an accelerated masters program to register for graduate-level classes which will count toward both their undergraduate and graduate degrees.
  • Personalized Minor form is used to approve a personalized minor which must include the required courses and a specific title for the minor. Upon receipt of the form, the courses are entered on the student's degree audit.
  • Reduced or Subsidized Tuition Class Request form is used by departments to request reduced or subsidized tuition for a class.
  • Senior Permission for Graduate Credit form is used to grant MSU seniors and visiting seniors approval to register for a graduate-level class.
  • Substitution, Waiver, or Advisor Approval Course List form is used to substitute a course required for a student's degree program with an alternate course or to submit an "advisor-approved" list of courses.
  • Temporary Clearance to Register form is used to temporarily override a Hold that is preventing a student from registering. This form will not remove the encumbrance.

  • Grade Change Authorization form used to submit a grade change. 
  • Incomplete Grade Extension form is to extend the deadline for completion of an incomplete grade. 
  • Instructor Drop form used to drop student from a class who has not attended by the second class meeting. 
  • Prerequisite Drop form used to drop student from a class who has an unmet prerequisite. 
  • Reporting Non-Attendance form used to report student who has stopped attending or never attended a class.
