Audit This form is used to place a course on "audit" status.
Authorization to Repeat This form is required for students who are exceeding the repeatable limit of a course, seeking special permission to repeat a "C" or higher grade, or pursue any non-standard course repeat.
Change of Schedule This form is used to initially register, add, or drop coursework. It should not be used to drop all remaining coursework (see withdrawal).
Degree Program, Application to This form is required for a student to be admitted to his/her major or minor degree program.
Duplicate Diploma Order This form is used to order a reissued and/or duplicate diploma. Current graduation candidates should not use this form unless they are planning to order an additional copy of their diploma.
FERPA Hold-Request To Prevent Disclosure of Student Information This form is used when students wish to prevent disclosure of any information from his or her record to University officials and others (including the student's advisor) except as authorized by FERPA.
FERPA Letter of Recommendation Form This form is used when students wish to allow the release of FERPA protected information for the purpose of securing a letter of recommendation.
FERPA Release of Information Authorization Form This form is used when students wish to request the release of FERPA protected information.
Grade Change Authorization This form is available only to faculty members who wish to submit a grade change. A secure login is required to access this form.
Grade Verification Form This optional form can be used by students who drop or withdraw after the no-penalty drop deadline.
Incomplete Grade Extension This form is available to instructors and departments for extending the deadline for completion of an incomplete grade. A secure login is required to access this form.
Instructor Drop This form is available only to instructors and departments for dropping students who have not attended by the second class meeting. A secure login is required to access this form.
Intent to Graduate This form is used to inform the University in what semester the student is planning to graduate. This form triggers the Office of the Registrar to check the student's degree audit to review graduation eligibility. Without this form, the student would not appear in the commencement program, receive a diploma, or appear on the University's graduation list. This Intent to Graduate form is for undergraduate students only. Graduate students should use the form located on the Graduate College web site.
Name Change This form is used when the student wants to change their official name at the University. Supporting documentation is required as outlined on the form.
Overload This form is used to secure permission to enroll in a number of credit hours that exceeds the University's limit for a semester/session.
Pass/Not Pass This form is used to place a course on a pass/not pass basis.
Permission, Course This form is used to grant a student permission to register for a closed and/or "permission required" course. The student should still complete a Registration form as well. This form gives permission to register but not does not complete the student's registration for the course.
Personalized Minor Form This form is used to notify the Office of the Registrar what courses have been approved for a student's personalized minor. Upon receipt of the form and admission into the minor, the courses are entered on the student's degree audit.
Prerequisite Drop This form is available to instructors and departments for dropping students who have an unmet prerequisite. A secure login is required to access this form.
Privacy Hold-Request To Prevent Disclosure of Student Information This form is used when students wish to prevent disclosure of his or her address, telephone number, and campus e-mail address.
Refund Exception Request Form This form is available to students who wish to file an appeal to the course refund schedule.

Reporting Non-Attendance Form This form is available to instructors for reporting students who have stopped attending or never attended a course.
Substitution, Waiver, or Advised Course List This form is used to substitute a course required for a student's degree program with an alternate course. It is also used if a course waiver is approved or if an advisor-approved list of courses is required.
Temporary Clearance to Register This form is used when an office/department wishes to override an encumbrance that is preventing a student from registering.
Transcript Request Form This form is used when a student wishes to request a transcript for themselves or to be sent to a third party.
Transfer Work, Change/Pre-Approval of This form is used when a department wishes to reevaluate a transfer course. It is also used when students want to seek pre-approval of how their transfer coursework will transfer back to Missouri State. This form is processed by the Office of Admissions.
Veterans Certification Request This form is used by veterans who are requesting for their enrollment to be verified for benefit purposes.
Withdrawal Request Form This form is used when a student wishes to withdraw from all courses for the semester.
