How to Request Enrollment Verification
Option 1
Use the self-service feature to print your own enrollment certificates and send to health insurers, credit issuers, housing providers, and others.
Option 2
A) For a standard Enrollment Verification Letter that contains only directory information, complete the online request form.
B) For an Enrollment Verification Letter that must include non-directory information, such as Student ID (SSN) or GPA, the student must complete the following steps:
- Print the Enrollment Verification Request Form page, in either Microsoft Word or fillable PDF
- Complete and fax, mail, or hand deliver the form to the Office of the Registrar. The fax number and mailing address is printed at the top of the form.
Option 3
Send a written request to: Office of the Registrar 320 Carrington Hall, Missouri State University, 901 S. National, Springfield, MO 65897. Requests must include the following information to be processed:

student's full legal name and any former names

student's social security number or student identification number

student's current address including day phone number

address or fax number to which verification is to be sent

student's signature to authorize the release of the non-directory information (if applicable)
Option 4
Students may request and pick up enrollment verification letters at the Office of the Registrar, Carrington Hall, Room 320, by supplying all of the above information plus photo identification. Verifications are typically completed within 24 hours.

Definition and Description
Verifications of enrollment status, graduation, and other information to lenders, employers, or insurance agencies are completed by the Office of the Registrar (320 Carrington Hall, 836-5520). Verifications are normally processed within 24 hours. The Office of the Registrar also participates in an enrollment verification program through an organization called the National Student Clearinghouse. The Clearinghouse is a source for instant verification of post-secondary student enrollment and graduation data. This service is paid for and utilized by lenders, guarantor, health insurers, credit issuers, employers, background screening firms, and other organizations so they can efficiently and accurately verify academic status of their clients. A standard enrollment verification letter will list the student's status (full-, half-, or less-than-half-time) along with the dates of the semester. Additional information, such as a parent's name or insurance ID number, or other types of verification (expected graduation date, enrollment history, etc.) may be added to a standard enrollment verification letter upon request.
The privacy of student information is protected under the Family Educational Rights and Privacy Act (FERPA). Therefore, Missouri State can release only selected information about a student, designated as "Directory Information," without the student's written consent (or as otherwise specifically allowed under FERPA). Refer to the Notification of Student Rights for a listing of the categories of information that have been designated as directory information at Missouri State University. As a student at Missouri State, if you wish to have non-directory information included in your enrollment verification letter, please print, complete, and sign a Request for Enrollment Verification Letter Form. This form may then be mailed, faxed or hand delivered to the Office of the Registrar for processing.
