Fee
There is a $20 charge (plus a 2.75% credit card fee) for each reissued diploma/certificate. Payment must accompany the request form. Missouri State University diploma covers can be purchased for an additional $10 each.

- Completed and signed Request for Reissued Diploma/Certificate form (fillable PDF Format). You need Adobe Reader to view and print this document.
- Payment by credit card, check or money order.
