Requirements for all CLINICS:
1. Cheer Tryout Release Form - Required for Clinic and Tryout Participation (must also be signed by a parent if under age 18), DUE AT THE FIRST CLINIC YOU ATTEND
2. Photocopy of your health insurance card (front & back) - Required for Clinic and Tryout Participation, DUE AT THE FIRST CLINIC YOU ATTEND
3. Copy of current physical signed by a doctor showing you are cleared for cheerleading, stunting, tumbling, running and strenuous activity - must be completed within the past six months - DUE AT THE FIRST CLINIC YOU ATTEND
4. Clinic Fee - due at each clinic (make checks payable to MSU)
