This form is to be used to submit those information technology proposals for review and/or funding which are "large-scale" or have the potential to impact the university’s technical infrastructure, other units, and/or technical personnel requirements. (If multiple proposals are being submitted, please indicate the relative priority of each.) This form and the associated approval/review process are intended to be informative in nature. Once the IT Council receives a completed form for review, it will endeavor to provide timely feedback so as not to hold up the project.
Once all required signatures have been received (via academic or non-academic route), please forward completed form to Administrative and Information Services, Carrington 104.
You have three options for accessing the Information Technology Proposal Form:
- Online Form
The online form allows you to enter all the requested information and generates a printable version. - Word Form
You can either print the Word form and fill it out by hand, or enter the information into the document and print the completed form. You need Microsoft Word to view and print this document. Make sure to click open, NOT save, when trying to view these Word documents. If you are asked for a username and password click Cancel. - Acrobat Form
You must print the Acrobat form and then fill it out by hand or on a typewriter. You need Adobe Acrobat Reader to view and print this document.
