The Jordan Valley Community Health Center sets itself apart from other clinics due to its collaborative work supported by a number of agencies, individuals and clinicians with a vision. This Center, which has been designated a federally qualified health center, serves as a health safety net to the southwest Missouri region and provides individuals with a medical home.

A medical home is defined as an approach to providing comprehensive primary care that facilitates partnerships between patients, providers and others as needed. The medical home model was developed to provide better access to health care. Understanding the needs of the community, the Jordan Valley Community Health Center exists to help traditionally underserved populations.
