Co-Curricular Funds: Expense Report

What is it?

This form is used to submit a request for Learning Community "co-curricular funds".

  • Each Learning Community cohort will have $125. 
  • Each cohort will select one teacher as the designated member to charge related expenses. 

Step 1—Submit this form to First-Year Programs for pre-approval. Kelly Wood, Interim Director, will email you your approval status.

Step 2—Purchase items and RETAIN RECEIPTS.

Step 3—Print Request for Co-Curricular Funds, sign and submit along with original receipts to First-Year Programs.

*This form will be forwarded to Financial Services for reimbursement.
