Veteran Certification Request Form
Form used to request School Certifying Official submit enrollment information to Department of Veterans Affairs. Complete each semester that a student wishes to receive education benefits.
Change in Status Form
Form used to report change in enrollment status (add/drop courses, withdrawal, change in major)
VA Parent Letter Request
Form used to request permission to enroll in a secondary school. All courses must transfer back to Missouri State and satisfy a degree or graduation requirement. This form is generally used for dual-enrollment, study away/abroad and for short term enrollment and other schools.
VA Cash Advance Application
Form used to request a cash advance for educational expenses. Cash advances will only be processed after the Change of Schedule Period (first week of classes) each semester.
Missouri State/MO S&T Engineering Degree Plan
Form used by students participating in the Cooperative Engineering programs with Missouri S&T and Missouri State. This form should be completed and signed by appropriate department personnel and will be used for GI Bill Certification as well as Tuition Assistance applications.

Direct Deposit Request (22-0246)
Form used to request DVA send monthly payment to direct deposit account
Tutorial Assistance Request (22-1990T)
Form used to request assistance with tutorial expenses. Please review the Tutorial Assistance criteria prior to submitting this form. Proper documentation from your instructor indicating need for individualized tutoring must be submitted along with this form.
