W-4 and Direct Deposit Completion Instructions for Departments

Federal W-4

  • Student must fill in all Biographical information.
    1. Full name (as it appears on their birth certificate or social security card).
    2. Permanent address (NO residence hall addresses)
    3. Social Security number
    4. Marital Status
  • Line #5 - Total number of allowances the student is claiming (from Line H or the worksheet on Page 2).
  • Line #6 - Does the student want extra money taken out of their paycheck each month
    If yes, the student may write in the dollar amount.
  • Line #7 - If the student is claiming EXEMPT, have them write EXEMPT on line 7.
    (No taxes will be taken out of their semi-monthly pay).
    If they have claimed exempt on line 7, lines 5 and 6 should be blank

Be sure they sign and date the form.
Do not fill out in pencil.

State W-4

  • Student must fill in all Biographical information.
    1. Full name (as it appears on their birth certificate or social security card).
    2. Permanent address (NO residence hall addresses)
    3. Social Security number
  • Line #1 - The student may enter an allowance for themselves (except if claiming EXEMPT). Skip to line #7.
  • Line #2 - If the student has a spouse you must complete this box.
  • Line #3 - If the student wishes to claim dependents, they need to enter the number of dependents claimed.
  • Line #4 - If the student wishes to claim additional dependents to lower their taxes, enter the number here.
  • Line #5 - Add lines 1-4 and enter total on line 5.
  • Line #6 - Does the student want extra money taken out of their pay each month?
    If yes, the student would write in the dollar amount. If no, they would leave it blank.
  • Line #7 - If the student claims EXEMPT, they need to write EXEMPT on line 7.
    ALL OTHER LINES MUST BE BLANK.

Be sure they sign and date the form.

Departments are not required to advise the student on how to complete their allowances. Each student must decide that for themselves. Just make sure that the forms are properly completed and signed. Students may return at a later date to fill out new W-4 forms to change names, addresses, or allowances.

Direct Deposit

  • Student must fill in biographical information.
    1. Full name (as it appears on birth certificate or social security card).
    2. Social Security number
    3. Telephone number (with area code)
    4. Name of Bank, Credit Union, or Savings and Loan
    5. Routing and Transit Number - routing and transit number are on the left bottom of their check and is always 9 digits.
    6. Account number
    7. Type of Account (Checking or Savings)

A voided check, copy of an account card, or letter from the student's bank with the above numbers (must be on bank letterhead) must be attached to form and student must sign and date.

If the student wants a portion of his/her pay to be deposited in a secondary account, the SECONDARY ACCOUNT portion of the form must be filled in and amount to be deposited must be indicated. The student must have routing and transit number and account number for this account also. A voided check, letter from bank (on bank letterhead), or copy of account card must be attached to the form also.

New student employees must fill out the Direct Deposit Form for Payroll even though they might have already done so online.
