In order to work on campus, you must fill out the following forms: I-9, W-4 and Direct Deposit. These forms are available in the department in which you were hired.
EMPLOYMENT ELIGIBILITY VERIFICATION FORM I-9
To complete the I-9 Form you must present proof of identity and proof that you are legally allowed to work in the United States. (Authority: Public Law 99-603 "Immigration Reform and Control Act of 1986")
You may choose any documentation from the following:

DIRECT DEPOSIT FORM
This applies to both checking and savings accounts.
In addition to completing the Direct Deposit Form, students must attach a voided check or one of the following:
ACCEPTABLE:

Copy of an account card from a bank, if it includes the 9-digit routing number

Something from the bank, either a form, or the information on the bank letterhead with the routing and account number (this can be faxed to the student, who brings it in)
NOT ACCEPTABLE:

Deposit slip
-
Check card
-
Bank statement
-
Passbook
You cannot complete the information from memory – there must be written documentation.
