As part of SOAR Carousel, all applicants must print and submit two (2) Faculty/Staff Reference Forms that have been completed by two different faculty or staff members. Recommendations from undergraduate students, such as Resident Assistants (RAs), will not be accepted. Graduate assistants may complete reference forms only if they serve as a direct work supervisor or course instructor for the applicant.
It is the applicant's--not the faculty or staff member's--responsibility to ensure that the completed forms are submitted on or before the deadline of 5:00 pm on Monday, October 31.
The Faculty/Staff Reference Form is available in two versions:
- The static version will need to be printed and completed by hand before submitted to the SOAR office.
- The fillable version can be completed on computer; it will then need to be printed and signed by the recommender, and submitted to the SOAR office.
