Download the Change of Address form

This form is used to request changes to:

• Temporary/Local address
• Primary permanent address

Bring completed form to the Office of the Registrar, Carrington Hall Room 320, or email scanned form to Registrar@MissouriState.edu from your Missouri State email account.

Students should update the following information in My Missouri State > My Profile channel > Personal Information:

• Personal email address
• Phone numbers
• Emergency contact individual

Employees must change their contact information with the Office of Human Resources.
