For Students in Selected Associate Degree RN Programs


Application to MSU:

Official Transcripts of all college course work sent to:

                                   Missouri State Admissions Office

Submission of Missouri State University Application for Admission Form

Cumulative GPA of 3.0 or higher with no course grades below a C

Submit the following directly to the Nursing Department:

 Completed Provisional Application for Scholars Program

Evidence of enrollment in Typhon

Evidence at AAS program of current:



              BLS CPR (AHA for health care providers)

              Background check with no restrictions

             Negative drug screen

Evidence of current health insurance

Recommendation from AAS Nursing Program Director

Academic Plan completed with MSU Nursing Department advisor

Student Disclosure Form


After Submission of above to MSU Nursing Department:

 Letter of PROVISIONAL BSN-C admission sent from:

                   MSU Department of Nursing to student

Attend Orientation on MSU campus

              Offered the First Monday before classes start each semester



Cumulative GPA of 2.5 or higher in all college course work attempted

Community College Transcripts containing graduation status submitted to MSU

Submit the following directly to the Nursing Department:

RN license without disqualification           

Evidence of tetanus vaccine (within 10 years)           

Evidence of Heb B vaccination for (series of 3)

                 Or immunity titer to Hep B

Evidence of vaccination for, Or immunity to, MMR

Proof of negative TB status (within 1 year)

                        Or medical follow-up        

Varicella (chicken pox) statement of past illness, titer, or vaccination

Evidence of current CPR Certification

                  AHA BLS for Healthcare Providers      

Evidence of Professional Liability Insurance  

Evidence of current health insurance

Criminal background check obtained by MSU Nursing Department

Drug screening obtained by MSU Nursing Department

Patient Safety Registry Registration online at http://health.mo.gov/safety/fcsr/

Complete the following

Blood Borne Pathogen yearly review

Receive Permission to continue: Letter from MSU Nursing Department
