The PCHC mission is to promote and protect the health and safety of Polk County citizens through the implementation of health services and programs based on assessed needs.
Communicable disease control, environmental services, family planning, tuberculosis control, sexually transmitted disease testing and treatment, pregnancy testing, Medicaid temporary card application, immunizations, WIC (Women, Infants, Children), health information services, vital records, child care consultation, worksite wellness, blood pressure screening, bioterrorism planning, local public health emergency mitigation.
No referral necessary for most services. Must be a Polk County resident. Administration of specialized treatment for tuberculosis and other medications require physician’s order.
The free services at the health center include childhood immunizations, tetanus shots, HIV testing and counseling, and confidential sexually transmitted infection screening and treatment. The tuberculosis skin test, Hepatitis A and Hepatitis B vaccines are provided for a fee. There are also services provided on a sliding fee schedule according to income and family size, which include pre-pregnancy counseling, pap smears and breast exams, pregnancy testing, and birth control methods.
The service providers at the PCHD include a nurse practitioner, two registered nurses, a nutritionist, a health educator, an environmental specialist, a regional planner for bioterrorism, a health professional assistant, and a health assistant.
