Mission
Hospice care is intended to meet the physical, emotional and spiritual needs of patients and to offer support for the family and significant others. Community Hospice of America provides care in the home and to residents of long-term care facilities. The goal of hospice care is to provide comfort to the patient by assisting with pain and symptom management and to enhance the quality of life for both the patient and family. In addition, this hospice program provides grief support to the family for one year following their loved one's death.
Services Provided
Nursing services, counseling, medical social services, home health, spiritual support, volunteers and bereavement support among others.
Population Served
All ages, color, race, religions, sex, national origin and the disabled all over the country. There is admission criteria that includes being diagnosed with a terminal illness, having a life expectancy not to exceed six months as determined by your physician, seeking palliative rather than curative therapy, it must be approved by your healthcare provider and it must be understood that artificial or life prolonging procedures are not consistent with the care received in hospice.
