This assignment is not actually a quiz.It was designed by the Kinesiology (KIN) Department to garner student demographic information and determine your entry level attitude and behaviors relative to exercise, nutrition and stress management.There are four assignments involved: one quick five question demographic survey, one 30 question wellness knowledge test, one 14 item attitude questionnaire and one 17 item wellness behavior questioinnaire. IN ORDER TO RECEIVE CREDIT (25 POINTS) FOR THIS ASSIGNMENT, YOU MUST COMPLETE ALL THREE SECTIONS TO THE BEST OF YOUR ABILITY! IN ADDITION, YOU MUST COMPLETE THE POST-TEST FOLLOW-UPS AT THE END OF THE SEMESTER TO RECEIVE CREDIT FOR THE PRE-TESTS. NO PARTIAL CREDIT WILL BE AWARDED. The actual questionnaire grades are ONLY indicators that you completed the assignments.
(1) To complete this process. click on your Laboratory Instructor's icon in the left column. This will link you to the InQsit quiz page. Click on the Radio Button, Demographics. Complete the 9 questions and submit your results.
(2) After completing the Demogrphics Survey, repeat the process for for PreTest A, the Wellness Knowledge Examination. Complete the 30 question test and submit your results.
(3) After completing the Wellness Knowledge Test repeat the process for PreTestB, the Wellness Attitude Survey.Complete the 14 item questionnaire and submit yur results.
(4) After completing the Wellness Attitude Survey, repeat the process again for PreTestC, the Wellness Behavior Survey.Complete the 17 item questionnaire and submit your results.
To receive ANY credit for this assignment, you must complete ALL FOUR sections of the PreTest. This assignment will be available from 12am, Monday, January 14th to 11:59pm, Thursday, January 31st.. You can only access the assignment one time. You will have 30 minutes to complete the wellness knowledge examination and 15 minutes to complete each section of the surveys..
