Missouri State University Wellness Program Incentives
Deadline extended to November 6, 2009 to earn savings!
The Missouri State University Wellness Program Incentives are available to complete online. All full-time employees are eligible to save up to $30 a month or $360 for the calendar year by completing the Education Component, Tobacco Use Statement, and the Health Risk Assessment (HRA) by October 31, 2009 Deadline extended to November 6, 2009. for 2010 health insurance premiums. All participation will be recorded electronically but employees can print off a Certificate of Completion for the Health Education Component and the Tobacco Use Statement for their records. For the Health Risk Assessment (HRA,) employees will receive an email from "MyHealthMedia" in their inbox once they have successfully completed the Health Risk Assessment (HRA) for their records. Also, it is not necessary to send any hard copy verifications to Taylor Health and Wellness Center or the Office of Human Resources since all participation is recorded electronically.
