Your Medical and Dental Claims Information

Effective March 1, 2006, as an employee, you have online access to your medical and dental claims history for the past three years through a new Med-Pay web page.  This information is password protected.

Follow this link to the Instructions for gaining access to your information.

  • Claims information for you and any member of your family who is a minor (under age 18) and who is covered through the University’s insurance plan(s) is shown under your name.
  • Claims information for your family member(s) who is 18 years of age or older (e.g., your spouse or adult dependent child) and who is covered through the University’s insurance plan(s) is not available to you without a signed release from him/her because of the Health Insurance Portability and Accountability Act (HIPAA) Privacy rules.
  • If your adult family member(s) wishes to view his/her claims information, he/she must provide a signed release form to Med-Pay.  The release form is available under theMedical Claim Statusoption on the Med-Pay web page’s menu.  To open a copy of the release form, click on the link “View Online Data Release Form for Adult Dependent” at the bottom of the page and print off a copy of the form.  When the form has been completed and signed, mail it to Med-Pay.  Once the form is processed by Med-Pay, you will be given access to all of your family’s claims information.
  • At this time, you are the only person who can access the Med-Pay web page and your adult family member only has access to the claims information using your ID and password after the release form has been processed by Med-Pay.  It is our understanding that in the future, your adult family member(s) will have direct access to their claims information.

Additionally, the web page has links to the Missouri State University medical and dental insurance booklets, St. John’s Health Network web page, and general information about health and wellness.
