Repair & Replacement Contract Form

This is an internal document to facilitate the flow and to document consensus of operating units within
Facilities Management on work scope and use of Maintenance Job Order Contract for Electrical, Plumbing, and/or Insulation. This document will be initiated by Facilities Maintenance, with review by the Director, and input from Design & Construction.

Upon final approval by the Assistant Director of Facilities Management, Maintenance will issue a notice to proceed to the appropriate contractor.
