Microsoft® Excel Required
To use this form, you need Microsoft® Excel. If you do not have Microsoft Excel on your computer, please complete one of the following options:
- Install Microsoft Excel through the Microsoft Campus License Agreement program. This program is available for University-owned computers or home computers of University technology users. For more details, see the Microsoft Campus Agreement page.
- Download the Excel Viewer software to your computer by following the instructions on the Microsoft Excel Plugins page.
