General Guidelines for Documentation

The Association on Higher Education and Disability (AHEAD) is the premiere professional association committed to full participation of persons with disabilities in postsecondary education. AHEAD has developed best practices regarding disability documentation in higher education. Missouri State's Office of Disability Services utilizes these best practices and the seven essential elements of quality disability documentation. Disability Services does not accept documentation hand-carried by the student requesting services. All documentation of disability must be provided to Disability Services directly from the appropriate professional and be presented on official letterhead from the professional, agency, clinic or organization providing the information.

Because it is the responsibility of the student to provide documentation supporting a disability and need for accommodations, Disability Services and Missouri State assume no financial responsibility for the acquisition of documentation.
