We would like to invite you to participate in the Fall 2008 commencement ceremony which is to be held on December 19, 2008 in the new JQH Arena. In order to facilitate your participation, the University will assume the cost of renting your gown. Our contract allows each participant to keep his or her cap and hood to use in future commencement ceremonies. If you plan to participate in the Fall 2008 commencement, please print this page, complete, and return it to the Dean of Students office by campus mail or fax (6-4134) no later than September 26, 2008.  The deadline for ordering regalia is September 26, 2008. We cannot process late requests.

 If you want the university to provide a hood for you, please read the "Regalia Hood Maintenance Assignment" information below and sign at the bottom of this form. Please direct any questions to the Dean of Students office, extension 65527.

 I, the undersigned, an employee of Missouri State University, agree to keep the hood, purchased by Missouri State, and turn in any other rental regalia (gown, cap, tassel) after commencement or freshman convocation.  I understand there is a value to the University to be able to avoid additional charges for inventory and maintenance.  Therefore, I agree to march in three commencement ceremonies.  I will be responsible for any cleaning and upkeep on the hood.  After I have marched in three commencement ceremonies, the cost of the hood, paid for by Missouri State, will be totally amortized, and it will become my property.  If I leave Missouri State and/or do not fulfill my responsibility for marching in three ceremonies I acknowledge that I owe, and will pay Missouri State for the balance of the hood cost not yet amortized.
