Please fill out the form and fax to Dixie Sullenger at 417-836-4756. Please have your order in by the second Friday of each month; as we pick up the supplies at the University of Missouri-Columbia on the first Friday of each month. Keep in mind the supplies are supplied to you at no charge with the agreement that you supply a designated driver a non-alcoholic drink in the CHEERS cup.
