The most recent feedback forms completed by the Assessment Council are available through the links on the left side of this page. Click on your college and then your department name to download your department's feedback form.

If you do not find your department listed it means that it has not been assessed by the Council in the last three years, so no file is available.

If you would like to use a blank feedback form as a guideline to make sure the assessment sections of your annual report are complete, the bottom link will take you to a page where you can download a blank feedback form.
