This Supplemental Application Form is only necessary if you do not qualify for admission under our selection index requirement and plan to apply for individual review. To be considered for admission, you must still complete the Undergraduate Admission application before or at the same time as you submit the Supplemental Application. Additionally, you will also need to pay the application fee (or submit a fee waiver from your high school) and submit any required transcripts and test scores.
Download and open the appropriate Supplemental Application Form as a Microsoft Word document. Complete, print, and mail the form to the Office of Admissions. If you do not have Word, you may view the Adobe Acrobat (PDF) version, and then copy and paste the questions into the word processing application you prefer. You will need to save this document on your computer so that you can print and mail it to us when it is complete.

You must submit the supplemental application form by June 1 to be considered for the summer and fall semesters and by December 1 for the spring semester.
